Rates and Insurance
Financial Policy
Payment is due at the time of service. There are no refunds for services.
Insurance
We accept the following health insurance plans and EAPs: Anthem, Optum, United Healthcare, Sentara, Optima, Aetna, Medicaid, and Medicare.
No Surprises Act
We are committed to complying with the No Surprises Act, which protects patients from unexpected medical bills. Our practice ensures that patients are informed about their rights and the costs of services. For those without insurance or choosing not to use their insurance, we provide a transparent estimate of the expected charges prior to delivering care.
Accessibility and Affordability
We believe in making mental health care accessible to everyone. To support this commitment, we offer a sliding scale fee structure based on each client’s financial situation, ensuring that our services remain affordable for all. See below for more details.
Cancellation Policy
We understand that life is full of unexpected situations. We also value our time and the ability to serve all clients effectively. If you need to cancel an appointment, please do so by leaving a voicemail with as much advance notice as possible.
To ensure fairness and maintain the integrity of our schedules:
- Late Cancellations: If you cancel with less than 24 hours’ notice, you may be charged up to 50% of the session rate.
- No-Shows: If you fail to attend an appointment without canceling, you may be charged up to the full session rate.
- Repeat No-Shows: If you miss more than one appointment without proper notice, we reserve the right to discontinue providing services.

Self-pay rates: without sliding scale
- Intake – $225
- Individual session:45 minutes – $150
- Individual session: 60 minutes – $175
- 90 minutes (EMDR only) – $250
- Family – 60 minute session – $200
- Couples – $175
- Group – Varies
Bundled services: pay in advance
(These bundles can be used for 60 or 90 minute sessions and must be used within 3 months)
- 3 hours – $450
- 4 hours – $500
- 6 hours – $700
Court Appearance Fee
The court appearance fee for our practice is $250 per hour, to include time spent preparing to appear in court with you or your lawyer.
Please note that our counselors do not participate in legal proceedings involving custody disputes. Additional rates will apply if travel out of Floyd County is required.
Sliding Scale Fee Structure
At Connected Counseling Co-op, we are dedicated to providing high-quality services to everyone. We understand that there may be times when insurance doesn’t cover a service, insurance coverage is lost, or other financial challenges arise, making it difficult to afford services. To address this, we offer a sliding scale option and encourage open and honest discussions with your clinician about your financial situation when these options are needed. We follow the Green Bottle Sliding Scale Method, a flexible yet structured tool for discussing sliding scale fees.
The Green Bottle method:
The Green Bottle Method refers to a system where individuals self-select their payment level for a service based on their financial situation. Imagine a green bottle filled with liquid representing different levels of financial security, labeled as “High Income,” “Medium Income,” and “Low Income”. This approach is intended to create a comfortable, structured process for folks to discuss finances and honestly assess their capacity to pay for services at each level.
How it works:
Developed by Alexis J. Cunningfolk, the Green Bottle Sliding Scale Method is designed to accommodate clients with varying financial means. This flexible payment range replaces a fixed price structure. Most services we offer are covered by insurance; however, sliding scale fees cannot be applied to copays, coinsurance, or deductibles, as doing so would constitute insurance fraud. The Green Bottle Sliding Scale Method is intended for those without insurance coverage, those who cannot use their insurance, or those whose insurance does not cover a service. Individuals can self-select the category and payment range that best aligns with their financial situation, with no need for financial verification. We trust our clients to be transparent and to inform us if their financial situation improves.
Creating a community of trust through transparency:
Transparency is crucial to our sliding scale policy. We openly share our sliding scale policy, how it is managed, and who can access it. We believe everyone deserves high-quality care, regardless of their ability to pay, while also valuing the investment in education, licenses, and learning that our practitioners have made. Although we cannot remove all barriers to care, we strive to make them as flexible as possible.
By adopting the Green Bottle Sliding Scale Method, we aim to promote a community of trust, understanding, and empathy. We want clients to feel valued and respected when they can choose a fee that aligns with their financial situation. This method also encourages clients to advocate for their needs, which can be empowering.
Full cost (pay 100% of Services):
If you have financial security, own property, or have personal savings, you would not qualify for the sliding scale. According to the ideals of the model, those who can afford “wants” and are not worried about securing necessities have economic privilege and power in our community.
Middle cost (pay 50%-75% of Services):
This range is for those who might struggle to pay full fees but do not face the financial hardships of those in the lowest cost category. These clients may have some debt, are trying to build or rebuild savings, and have steady income but live paycheck to paycheck without concerns about basic needs like shelter and food. These folks may need to seek financial help, especially for ongoing therapy or services.
Bottom cost (pay 15%-30% of Services):
This category is for those who cannot afford therapy without significant discounts. We offer no more than two slots at the bottom cost per clinician to maintain our practice’s financial health. Individuals in this category are those struggling to access basic needs like healthcare, housing, food, and child care, and who live paycheck to paycheck or are in significant debt.
